ABSTRACT

OBJECTIV E To develop the infor mation-based pharmaceutical care pathway of anticoagulant therapy in patients with atrial fibrillation and improve the efficacy and safety of treatment for them. METHODS The“anticoagulant pharmaceutical care”module was developed on the basis of medical intelligent and decision system. Patients with atrial fibrillation were taken pharmaceutical care in the whole anticoagulant treatment by evaluating the thromboembolism and bleeding risks ,pre-reviewing antithrombotic prescriptions ,monitoring efficacy and drug interactions ,and warning adverse reactions. RESULTS A total of 1 228 patients receiving anticoagulant therapy were enrolled. It was found after analysis of their doctor ’s orders that 9.27% of the patients adjusted the improper antithrombotic therapies ,3.99% modulated treatments according to the effects of potential drug interactions or the risk of adverse reactions ,and 70.29% of the wrong prescriptions were intervened successfully. After the information-based pharmaceutical care ,the anticoagulation treatment rate increased from 88.73% to 97.40%,the rate of patients ’achievements to warfarin’s international normalized ratio in hospital increased from 38.64% to 66.67%,and the incidence of serious bleeding events decreased from 2.94% to 0.37% (P<0.05). CONCLUSIONS The information-based pharmaceutical care path of anticoagulant therapy achieved comprehensive ,efficient and accurate management of patients with atrial fibrillation ,and improved the rationality ,effectiveness and safety of anticoagulant therapy.

ABSTRACT

Ornithine decarboxylase (ODC) is a key enzyme in the biosynthetic pathway of polyamines and catalyzes the decarboxylation of ornithine to produce putrescine. Inhibition of ODC activity is a potential approach for the prevention and treatment of many diseases including cancer, as the expression levels and the activities of ODC in many abnormal cells and tumor cells are generally higher than those of normal cells. The discovery and evaluation of ODC inhibitors rely on the monitoring of the reaction processes catalyzed by ODC. There are several commonly used methods for analyzing the activity of ODC, such as measuring the yield of putrescine by high performance liquid chromatography, or quantifying the yield of isotope labelled carbon dioxide. However, the cumbersome operation and cost of these assays, as well as the difficulty to achieve high-throughput and real-time detection, hampered their applications. In this work, we optimized a real-time label-free method for analyzing the activity of ODC based on the macromolecule cucurbit[6]uril (CB6) and a fluorescent dye, DSMI (trans-4-[4-(dimethylamino) styryl]-1-methylpyridinium iodide). Finally, the optimized method was used to determine the activities of different ODC inhibitors with different inhibition mechanisms.

ABSTRACT

Objective:To analyze the surgical methods of operable breast cancer and analyze the follow-up results.Methods:A retrospective analysis of the clinical and pathological data of 636 operable breast cancer patients admitted to Zibo First Hospital from July 2008 to April 2018, including the clinical stage, pathological staging. Analyze of the proportion of four surgical methods, and through follow-up, analyze the treatment effect of different surgical methods.Results:All patients are female, aged 26-80 years, the clinical stage of 636 patients: Tis 18 cases, stage Ⅰ 143 cases, stage Ⅱ 354 cases, stage Ⅲ 114 cases, stage Ⅳ 7 cases. There are four types of surgery: ① breast conserving surgery + sentinel lymph node biopsy in 124 cases (19.50%); ② breast conserving surgery + axillary lymph node dissection in 39 cases (6.13%); ③ mastectomy + sentinel lymph node biopsy in 163 cases (25.63%); ④ modified radical surgery in 310 cases (48.74%). Sentinel lymph node biopsy in 427 cases (67.14%), success in 404 patients (94.61%); all patients with lymph node negative 384 cases (60.38%). Follow-up for 1 to 9 years, 11 cases of local recurrence after breast-conserving surgery, It accounted for 6.75% of breast-conserving surgery; 43 cases of local recurrence of chest wall after mastectomy, accounting for mastectomy 9.09%; 33 cases of recurrence and metastasis of axillary lymph nodes and supraclavicular lymph nodes, 4 cases of axillary recurrence after sentinel lymph node biopsy.Conclusions:The proportion of breast-conserving surgery in this group of patients was high and the local recurrence rate of breast-conserving surgery was less than that of mastectomy group; the proportion of simple modified radical surgery declined further; patients with axillary lymph node metastasis were less in the whole group. The choice of reasonable operation method is an important factor to improve the prognosis of breast cancer.

ABSTRACT

OBJECTIVE:To investigate the correlation between the morphological characteristics and chemical constituents of garden ginseng (GG),mountain transplanted ginseng (MTG)and mountain-grown ginseng (MGG),so as to provide evidence for elucidating the scientific connotation of “evaluating quality from morphological characteristics ”. METHODS :A total of 30 batches of GG ,MTG and MGG were collected. The contents of 11 saponins(ginsenoside Rg 1,Re,Rg2,F1,Rb1,Rb2,Rb3,Rc,Rd,Ro, Rf)were determined by UPLC. The determination was performed on Acquity UPLC ® BEH C 18 column with mobile phase consisted of 0.1% Formic acid solution- 0.1% Acetonitrile formate solution (gradient elution )at the flow rate of 0.5 mL/min. The column temperature was set at 35 ℃. The detection wavelength was set at 203 nm,and sample size was 5 μL. The content of starch was determined by colorimetry ,and the absorbance value was measured at 559 nm with 0.5% iodine-potassium iodide as the color reagent. SPSS 23.0 statistical software was used to perform Pearson relationship analysis for appearance characteristic indexes (number of stem scars ,rhizome length ,texture)with the contents of 11 kinds of saponins ,the content of total saponins ,the content ratio of panaxatriol saponin (PPT)/panaxadiol saponin (PPD),the content of starch. RESULTS :The linear relationship between UPLC and colorimetry was good (r>0.999). RSDs of precision ,reproducibility and stability tests were all lower than 5% . Average recoveries were 95.17% -105.20%(RSD<5% ,n=6). Except for ginsenoside Rf ,the contents of other 10 ginsenosides were positively correlated with the number of stem scars ,so were the contents of 11 gensenosides with the rhizome length(r>0,P<0.05),and they were negatively correlated with the firmness of the texture (r<0,P<0.05). The content of starch in P. ginseng was positively correlated with texture firmness (r=0.95,P<0.01). The ratio of PPT/PPD in P. ginseng was negatively associated with the number of stem scars and the rhizome length (r<0,P<0.05). CONCLUSIONS :The number of stem scars ,the rhizome length and texture porosity of P. ginseng are positively associated with the contents of medicinal material ginsenoside;the increase of PPD content in P. ginseng was higher than that of PPT with the increase of growth years. The traditional“evaluating quality from morphological characteristics ”for P. ginseng has a certain scientific connotation ,which is more stem scars ,longer rhizome and loose texture.

Objective@#To explore the effect of grain-moxibustion combined with routine western medicine therapy on the quality of life of the patients with secondary hyperparathyroidism after hemodialysis.@*Methods@#According to the random number table, 80 hemodialysis patients with secondary hyperparathyroidismin were divided into two groups, 40 in each group. The control group was treated with routine western medicine therapy, while the observation group was treated with grain-moxibustionon the basis of the control group. Both groups were treated for 3 months. Clinical curative effect was evaluated after treatment. The quality of life was assessed by SF-36 Health Survey (SF-36) before and after treatment, serum Ca, P and ALP levels were measured by colorimetry, and intact parathyroid hormone (iPTH) was detected by double antibody sandwich ELISA.@*Results@#Total effective rate was 95.0% (38/40) in the observation group and 80.0% (32/40) in the control group, where there was significantly difference between two groups (χ2=4.114, P=0.043). After treatment, the levels of serum Ca (2.27 ± 0.18 mmol/L vs. 2.07 ± 0.16 mmol/L, t=5.252) in the observation group was significantly higher than that in the control group (P<0.01), the levels of serum P (1.32 ± 0.02 mmol/L vs. 1.60 ± 0.04 mmol/L, t=39.598), iPTH (210.15 ± 16.24 pg/ml vs. 325.41 ± 21.55 pg/ml, t=27.015) and ALP (71.14 ± 10.73 U/L vs. 116.42 ± 12.26 U/L, t=17.577) were significantly lower than those in the control group (P<0.01). After treatment, the scores of physical health, body role function, body pain, overall health score, middle energy, social function, emotional role function and mental health in the observation group were significantly higher than those in the control group (t=2.399, 3.572, 2.684, 3.598, 3.076, 2.811, 4.958, 2.176, P<0.01).@*Conclusions@#Grain-moxibustion combined with routine western medicine therapy in treatment of hemodialysis patients with secondary hyperparathyroidism can adjust the calcium and phosphate metabolic disorders, reduce the parathyroid hormone levels, and they improve the quality of life of patients.

ABSTRACT

OBJECTIVE: To compare low-polarity volatile constituents in supercritical CO2 extract from the roots and stem of Ilex asprella and its effects on the proliferation of IEC-6 in vitro, and to provide reference for making full use of wild resources of I. asprella and expanding its medicinal parts. METHODS: The low-polarity volatile constituents were extracted from the root and stem of I. asprella with supercritical fluid CO2 extraction(SFE-CO2). The chemical constituents were analyzed by GC-MS. IEC-6 cells were treated with different concentrations of supercritical CO2 extracts (0, 1, 5, 10, 20, 40, 60, 80, 100 μg/mL) from roots or stems of I. asprella. MTT assay was used to detect the relative viability, and cell proliferation curve was drawn and EC50 of each extract were calculated. RESULTS: Sixty-two and forty-six low-polarity volatile constituents were identified from supercritical CO2 extract in the roots and stem of I. asprella with GC-MS; there were 24 common constituents totally, mainly including pelargonic acid(14.18% and 6.14%),octanoic acid(10.59% and 4.35%),hexanoic acid(8.63% and 10.86%),paeonol(7.79% and 6.00%),2-methyl-3-phenyl-propanal(6.3% and 0.58%),acetic acid(1.72% and 33.77%) in root and stem, respectively. The results of cell culture in vitro showed that when the concentration of supercritical CO2 extract from the roots and stems of I. asprella was lower (≤60 μg/mL), it could significantly promote the proliferation of IEC-6 cells and their EC50 were 16.35, 20.20 μg/mL, respectively; when the concentration of the extract was higher (≥80 μg/mL), it showed cytotoxicity and inhibited the proliferation of IEC-6 cells. CONCLUSIONS: There are similar species of volatile constituents in roots and stems of I. asprella and similar in vitro bioactivity of the supercritical CO2 extracts to IEC-6 cells. The short-chain fatty acids may be the active ingredient to promote cell proliferation, while paeonol may be the cytotoxic active ingredient.

ABSTRACT

Objective:To explore the related influencing factors of carotid atherosclerosis(AS)plaque formation and stability in the patients with type 2 diabetes mellitus(T2DM),and to provide scientific evidence for early prevention and treatment.Methods:A total of 249 cases of simple T2DM patients were selected.According to the results of carotid artery color Doppler ultrasound,they were divided into AS group,stable plaque group and unstable plaque group.The differences in physicochemical indexes and life style of the patients were compared between three groups;univariate analysis and Logistic regression analysis were used to screen the related influencing factors of carotid AS plaque formation and stability of the T2DM patients.Results:The univariate analysis showed that age,fasting blood glucose(FBG),glycosylated hemoglobin(HbA1c),non-high density lipoprotein cholesterol (NHDL-C),monocyte/ high density lipoprotein ratio(MHR),smoking ratio(Smoking%),T2DM disease course,high density lipoprotein protein cholesterol(HDL-C)had significant differences between AS group and plague group(P<0.05).There were significant differences in the values of postprandial blood glucose(PBG), systolic blood pressure(SBP),low density lipoprotein cholesterol(LDL-C),HbA1c,MHR,Smoking% and HDL-C of the patients in stable plaque group and unstable plaque group(P<0.05).The Logistic regression analysis showed that high age,HbA1c,NHDL-C,MHR and smoking were the risk factors of the plaque formation and high HDL-C was a protective factor of plaque formation(Age:OR=1.62,P=0.011;HbA1c:OR=1.25, P=0.027;HDL-C:OR=0.65,P=0.009;MHR:OR=3.50,P=0.000;Smoking:OR=2.28,P=0.009;NHDL-C:OR=1.39,P=0.028).High SBP,LDL-C,MHR and smoking were the risk factors of unstable plaque formation,and high HDL-C was a protective factor of unstable plaque formation(SBP:OR=1.5 7,P=0.003;LDL-C:OR=1.99,P=0.000;MHR:OR=3.88,P=0.000;Smoking:OR=2.01,P=0.001;HDL-C:OR=0.53,P=0.001).Conclusion:For the patients with simple T2DM and carotid AS plaque,blood lipid,blood pressure and smoking cessation should be emphasized and HDL-C level should be increased,which can effectively prevent the formation of AS plaque and stabilize the plaque.

ABSTRACT

OBJECTIVE@#To explore the relationship between serum cystatin-C (Cys-c) levels and vibrating perception threshold (VPT) in patients with Type 2 diabetes mellitus (T2DM).
@*METHODS@#According to the symptoms, signs and results of lab examination, a total of 352 patients with T2DM were divided into a diabetic peripheral neuropathy group (DPN group, n=107) and a non-diabetic peripheral neuropathy group (NDPN group, n=245). Serum Cys-c levels were measured by radioimmunoassay method. The relationship between serum Cys-c levels and VPT, estimated glomerular filtration rate (eGFR), urinary albumin-to-creatinine ratio (ACR), glucose and blood pressure and other parameters were also analyzed by correlation and multiple regression analysis. All T2DM patients were divided into a high Cys-c levels group (n=89) and a low Cys-c levels group (n=263) according to the upper quartile of Cys-c, and the incidence of DPN and VPT levels in each group were compared. Risk factors of DPN in T2DM patients were analyzed by binary logistic regression analysis and receiver operating characteristic (ROC) curve was used to identify the optimal cutoff of serum Cys-c levels for predicting DPN in patients with T2DM.
@*RESULTS@#Serum Cys-c levels were significantly higher in the DPN group than that in the NDPN group [(1.04±0.43) vs (0.80±0.25) mg/L, P0.996 mg/L, the predicts have high risk of DPN in T2DM patients, which might be related to diabetic nephropathy, oxidative stress and inflammatory reaction induced by advanced age, hyperglycemia and hypertension.

ABSTRACT

Objective To investigate the effects of ketogenic amino acid ( KAA) replacement diet on insulin resistance in mice fed with high fat diet(HFD) and to analyze the possible mechanism. Methods C57BL mice were fed with a control diet, HFD, and KAA-fortified HFD(HFDKAAR)from the age of 8 weeks, and 8 weeks after HFD initiation, the HFD-fed mice were divided into two groups:one group of mice were fed the same HFD, the other group were fed HFDKAAR ( HFD→HFDKAAR ) . The metabolic evaluations were performed at the end of 16 weeks. Blood glucose levels were measured at 0, 15, 30, 60, 90, and 120 min after the injection of glucose ( 1 g/kg BW intraperitoneal glucose tolerance test, ipGTT) . The insulin,β-hydroxybutyrate, and acetoacetate levels in the plasma were measured via ELISA. The insulin resistance index ( IRI) and area under curve ( AUC) were calculated. The expression of hepatic LKB1 ( liver kinase B1 ) , AMP-activated protein kinase ( AMPK ) , and mTOR ( Mammalian target of rapamycin ) protein, and mcp-1 mRNA were measured by western blot and real-time PCR respectively. Results HFD-fed group of mice displayed significantly heavier body weight,heavier intra-abdominal fat weight, and significant deterioration of glucose tolerance at the end of 16 weeks in addition to higher insulin levels( all P<0. 05), HFDKAAR-fed mice exhibited significantly ameliorated high fat diet-induced obesity and glucose intolerance compared to the HFD-fed mice, which was associated with decreased insulin levels, IRI, AUC, and mcp-1 mRNA expression (all P<0. 05). HFD suppressed hepatic LKB1 and AMPK phosphorylation expression, and increased mTOR phosphorylation levels compared to the control diet-fed mice(all P<0. 05). In contrast, treatment with the HFDKAAR diet increased LKB1and p-AMPK expression, which was associated with suppressed p-mTOR levels compared to the HFD-fed mice(all P<0. 05). Conclusion KAA may ameliorate high fat diet-induced obesity, glucose intolerance, via normalizing the hepatic LKB1-AMPK-mTOR nutritional signal passageway. KAA replacement diet seems to be a potential nutritional intervention for the treatment for patients with metabolic defects, such as obesity, glucose intolerance, as well as metabolic syndrome.

Objective To study the optimized process for effective components in Danggui-Buxue pill by orthogona design,and provide the basis for the establishment of quality standard.Methods Ferulie acid and ligustilide were used as examining index,the extraction time,solvent volume and extraction times were used as factors.L9(34) orthogonal test table was used to determine the optimum extraction process.HPLC method was used to establish methodology for ferulie acid and ligustilide.Results According to the result of orthogonal design,comprehensive consider determining the extraction efficiency of ferulic acid and ligustilide the optimal extraction technology as follows:1.0 g Danggui-Buxue pill was extracted by 30 ml of methanol for 60 min,extraction times for 2 times.Conclusions This method is simple with high efficiency.Its HPLC method has high precision and good reproducibility,which can be used for analysis Danggui-Buxue pill.

<p><b>OBJECTIVE</b>To study the pyrrolizidine alkaloids of Senecio cannabifolius var. integrilifolius.</p><p><b>METHOD</b>Various chromatographic methods were used for the isolation and purification of chemical constitutents of the plant and their structures were elucidated on the basis of spectral analysis.</p><p><b>RESULT</b>Four compounds were determinated as jacobine(1), jacoline(2), jaconine(3), senecicannabine(4).</p><p><b>CONCLUSION</b>All compounds were isolated from this plant for the first time.</p>

ABSTRACT

ObjectiveTo evaluate the clinical effect of ultra pulse CO2 laser in treatment of 32 patients with primary amyloidosis of the skin. Methods64 patients with primary amyloidosis of the skin were selected and randomly divided into two groups,the observation group(32 cases)and the control group(32 cases).The observation group were treated with ultra pulse CO2 laser,and the control group were treated by routine medicine method.Clinical effect were compared between the two groups after twelve weeks therapy. ResultsThe total effective rates of the observation group and the control group were 90.6%,53.2% respectively.There were a significant difference between the two groups(x2 = 11.13,P<0.01).There were some different irritations in two groups after therapy,to give symptomatic treatment could help relieve.The relapse rate in the observation group was significant lower than that in control group after treatment of 3 ~ 6 months(x2 = 9.471,P<0.01). ConclusionUltra pulse CO2 laser therapy of primary amyloidosis of the skin was safe and effective.

Senecio cannabifolius var integrilifolius (Compositae), locally known as "Fanhuncao" in China, is a folk herb used for the treatment of pneumonia, virus influenza and bronchitis. To investigate the chemical constituents of this herb, water extract of the aerial parts was subjected to various chromatography on normal/reversed phase silica gel and Sephadex LH-20 column. Eleven compounds were obtained and identified on the basis of their physicochemical properties and spectroscopic analysis as senecine (1), p-hydroxy-benzeneacetic acid (2), protocatechuic acid (3), 2,5-dihydroxy-benzeneacetic acid (4), 3,4-dihydroxy-benzeneacetic acid (5), vanillic acid (6), caffic acid (7), succinic acid (8), 2-furoic acid (9), 1,2,4,5-tetrahydro-jacaranone (10), and 4-(pyrrolidin-2-one)-phenylacetic acid (11). Compound 1 was structurally identified to be a new compound;the other compounds were isolated from this plant for the first time.

ABSTRACT

Objective To explore the effect of antibiotics to neonatal intestinal microflora.Methods the neonatus who had infectious pneumonia were selected as case-groups,and divided into three sub-groups according to the antibiotics the patient used randomly.Tak- ing the period when all indexes were normal as pneumonia being cured,and curing rate and time-effect were compared among groups.To explore the effect of antibiotics to neonatal,the main microflora which can reflect the all intestinal microflora were quantitatively analysed when antibiotics had been used three and seven days respectively.Results The results showed that:①there is no significant difference on curing rate and time-effect among the three groups;②antibiotics showed effect to neonatal intestinal microflora,and the longer the antibi- otics was used,the more significant the effect was;③the group with penicillin and cynnematin simultaneously showed the most effect to in- testinal microflora,and the group with cynnematin only showed moderate effects on intestinal microflora,and the group with penicillin showed little effect to intestinal microflora.

ABSTRACT

Objective To observe the therapeutic efficacy of combination of smecta and P.O.Bifidobigen in treatment of infantile diarrhea .Methods 100 cases with infantile diarrhea were collected randomly. 6 2cases were treated with combination of smecta and P.O.Bifi dobige n,while 62 cases were treated with s mecta alone .Results 62 cases(62%)showed an obvious efficacy ;30 cases(30%)did effective;8cases(8%)showed no effective .The total effective rate was 92.4%.For infectious and symptomatic diarrhea,there was not difference of effective rates between the two therapies.For digestive dysfunction and chronic diarrhea accomplished with malnutrition,just the combination of the two drugs showed efficacy .Conclusions For infantile diarrhea,the combination of smecta and P.O. Bifidobigen can result in efficacy in addition to controlling its causes.

ABSTRACT

Purpose The aim is to prepare the extract of immunocompetent cell proliferation from human placenta and to try to find out a suitable method of preserving the extract.Methods The extract was mainly prepared by heat-treating placenta homogenated fluid. Then the activity to stimulate murine splenic lymphocyte proliferation in vitro was done with MTT , exposing the extract to radioisotope 60 Co.Results The content of protein was 2~3mg per gram placenta measured by Lowry′s Method.The rate to promote cell proliferation was more than 80 percent.The activity lasted a few months after being exposed to radioisotope.Conclusion The extract prepared by heat-treating not only had high activity, but also had the unique method and good repeatability.This prepared extract as a kind of stable,reliable and remarkably promoting lymphocyte proliferation reaction had the value of development production on broad scale as well as in clinical practice.

ABSTRACT

Objective To evaluate the reliability and sensitivity of detection of thyroid carcinoma lymph node micrometastasis with MUC1.Methods In 488 cases of thyroid disease,1% methylene blue 1.0-2.0mL was injected into the thyroid nodule or surrounding thyroid tissue 24hours before operation.The dye-stained lymph nodes were detected at operation and RT-PCR method was used to determine MCUI in the clinically blue-stained lymph nodes.Results The detection rate of the blue-stained lymph nodes in thyroid carcinoma was 93%,and in benign lesions was 0%.In the research group,80 cases were found to have blue-stained lymph nodes,the detection rate was 95% and was higher than the pathologr diagnostic rate which was 86%(P

ABSTRACT

Objective To study the correlation of p53,ki-67 and bcl-2 expression in breast cancer with the clinical response to neoadjuvant chemotherapy,and search for biological markers to guide therapy and predict prognosis.Methods The expression of the p53,ki67 and bcl-2 was examined by immunohistochemical(assay) in samples of breast cancer tissue which taken from 118 cases of operable breast cancer patients.The relationship between p53,ki-67 and bcl-2 and the effect of neoadjuvant chemotherapy was analyzed by(chi-square) test.Results The overall response rate to neoadjuvant chemotherapy was 68.6%.Patients with expression of ki-67 were more likely to respond to treatment.The effect of neoadjuvant chemotherapy had a significantly negative correlation with p53 expression.There was no significant difference between the response to neoadjuvant chemotherapy and bcl-2 expression.Expression of p53 protein was correlated with low(expression) of bet-2.Expression of p53 and ki-67 protein was significantly related.Conclusions The(results) indicated that p53 and Ki-67 expressions are strong prognostic molecular markers that can be a guide for neoadjuvant chemotherapy and prediction of prognosis for patients with breast cancer.
